With us, we work together to create safe and well-functioning solutions for people in need of support. We are now looking for a social worker to our team who wants to make a difference through a professional and caring approach.

The team consists of 10 social workers, 5 nurses, 3 occupational therapists, 3 physiotherapists, and an assistant – a cross-professional group with broad competence and diverse experiences.

Job Responsibilities
As a social worker in the care planning team, you are responsible, together with your colleagues, for authority decisions according to the Social Services Act upon discharge from inpatient care and for follow-up of short-term decisions.
Your daily work as a social worker includes investigation, decision-making, and authority exercise. This means that you will analyze individual needs and make well-founded decisions about appropriate interventions after discharge from hospital and short-term housing.
In your role as a social worker, you collaborate in a team with other professional groups, both internally and externally.

Qualifications
We are looking for someone who has:
- Degree in Social Work (210 credits)
- At least one year of experience in authority exercise
- Good knowledge of applicable legislation and practice, as well as experience in documenting and justifying decisions in a clear and legally secure manner
- Experience using a computer as a working tool and a willingness and ability to quickly familiarize yourself with new programs and systems
- Very good knowledge of Swedish, both spoken and written
It is advantageous if you have experience in:
- SVU process and follow-up of short-term decisions
- Cross-professional work
- Operational systems Lifecare and Mina planer

About the Workplace
The Authority Department in the Health, Care, and Social Services Administration is organized into three units, one of which is responsible for collaboration and planning upon discharge from inpatient care and follow-up of short-term decisions. In everyday language, we call ourselves SVU, an abbreviation of collaboration upon discharge from inpatient health care. The unit consists of about 80 employees divided into three sections with cross-professional teams including social workers, nurses, occupational therapists, physiotherapists, and assistants. The professions in the team decide based on their assignments on the need for interventions according to the Social Services Act (SoL) and interventions according to the Health and Medical Services Act (HSL).
